Glass effects in Photoshop...

Does anyone know how to pull these off correctly?

I haven't been able to find a decent tutorial, that worked, yet.

I've tried most of this on my own. Attempts at what I thought might be able to produce it...

I only saved one of my attempts. Did I do it right?

PLEASE Help me, if you can. :)
